CERVICAL CANAL STENOSIS C2-3, 3-4 WITH MALACIA OF THE SPINAL CORD.
Anamnesis
The patient came to the clinic
09-May-2016 complaining of neck and left
shoulder pain for 3 weeks with ataxic gait
last 2 days. The patient is a known diabetic for
8 years, hypertensive for 6 years, underwent
open heart surgery 12 years ago, in L-thyroxin
100 microgram for 15 years and in Baby aspirin.
On examination: Romberg position was normal with
rigid unmovable neck. Weak extension right hand
4/5, 3/5 left hand and 4/5 both triceps muscles.
The patient was sent for thorough investigations
and MRI of the
cervical spine
showed severe
stenosis C2-3, C3-4 with malacia of the spinal
cord. Dynamic studies showed ankylosing
spondylitis with fusion of C3 down to the
thorax. The patient was sent for cardiologic
evaluation and concor was given for RBBB.
Decompressive laminectomy of
C2,3,4 was performed after drilling and thinning
of the laminae using high speed drill and
turning the flap en one block The dura was
transparent. Routine closure of the
wound.
Smooth postoperative recovery. The
power of both upper limbs became normal. He was sent to the ward.
Comments
There stenosis is a progressive one and
the sooner the surgical intervention, the better the
outcome.
