22-JANUARY-2005 BAHIYEH ELIAS YOUSEF 80 YEARS LCS 3-4-5 WITH FORAMINAL STENOSIS
LEFT L4 ROOT.
Anamnesis
The patient came to the clinic 17-January-2005
complaining of LBP for 2 months with
bilateral sciatica, more the left. She can walk
only 400 meters with intermittent claudication.
She is a known hypertensive for 20 years.
On examination: The patient is limping with exaggerated scoliotic stance.
There is weak dorsiflexion left foot -4/5. SLRS was
70 degrees with pain left side.
The patient was sent for investigations and MRI
of the lumbar spine performed 18-January-2005
showing severe lumbar canal stenosis L3-4 and
L4-5 with foraminal compression left L5 root.
The patient was sent to cardio consultation.
Extended decompressive
laminectomy L3,4 and lower half of L2 and upper half
of L5. Foraminotomy both L4 and L5 roots. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the left foot improved.
She was sent to the ward.
Follow Up
The patient came to the clinic 02-February-2005
with clean wound and neurologically free.
Comments
The earlier surgical intervention, the
better the outcome, because LCS is an progressive one.
Skyra MRI with all clinical applications in the run since 28-Novemeber-2013.
