12-JANUARY-2013 BALASEM HAMADAH GAMOOS 33 YEARS HUGE
EXTRUDED DISC L5-S1 WITH LEFT POSTERIOR MIGRATION.
Anamnesis
The patient came to the clinic 12-January-2013 complaining of LBP
and left sciatica and pain in the left perianal
region for 8 months with
exacerbation of the sciatica the last moth
reaching the left S1 territory with numbness and
pain IV and V toe left foot.
On examination: the patient
in agonizing pain. He is limping with
exaggerated scoliotic stance. SLRS 40 degrees
right side with pain and 5 degrees with more
pain in the left. There is weak dorsi 3/5 and
planterflexion
-4/5 left foot with absent left AJ.
MRI done 12-January-2013
showing huge extruded disc L5-S1 with left
posterior and downward migration.
Left S1 foraminotomy. There is no epidural fat
around the root due to severe compression. The
extruded disc was removed lateral to the axilla.
Left sided cleaning L5-S1
Routine closure of the wound.
Smooth postoperative recovery.
The power of the left foot became normal.

Comments
The patient still have an
estimated postoperative recurrence around 7%,
because the disc space is not completely
shallow.
