03-SEPTEMBER-2015 BASSAM HASAN AL-QASSAS 53 YEARS
EXTRUDED DISC C7-D1 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 29-August-2015
complaining of severe right shoulder pain for 5
days making him unable to sleep. He cannot turn
his head to the right. The patient was operated
by me in December-2003 for PCD C6-7.
On examination: the patient was in agonizing
pain with weak grip and extension right hand
with weak right triceps muscle. He has short
neck, obese diabetic and hypertensive. He has no
TOS nor frozen right shoulder.
The patient was sent for investigations. MRI of
the lcervical spine performed 31-August-2015
showing extruded disc C7-D1 with right foraminal
occlusion.
Diskectomy C7-D1 was
performed. Considering the short neck and deep
fatty wound, the operation was difficult and the
instrumentations and angle of vision were all
challenging. It was impossible to identify the
level in Lateral view and the level was
identified by the level f the previous surgery.
A cage 6 mm with Atlantis one level 27.5
mm length and 4 screws 15 mm, fusion of C7-D1
was achieved. Routine closure of the
wound.
Smooth postoperative recovery. The power of
right upper limb became normal, but the shoulder
pan still the same. The patient was
sent to the ward.
Comments
The patient is a challenging case for
surgery. Short neck obese and difficult angle of work.
