HIJAB AL-FAYEZ 40 YEARS RECURRENCE OF HIGH GRADE ASTROCYTOMA RIGHT MEDIOBASAL
FRONTO-TEMPORAL LOBES.
Anamnesis
The patient operated by me
22-Npvember-1999 for right insular high
grade astrocytoma and showed improvement and
check MRI showed no recurrence and he underwent
radiotherapy at Al-Bashir hospital. The patient
then came to the clinic 05-November-2003 with
new MRI done the same day showing huge
recurrence of the tumor and he was advised to
re-operation.
On examination: The patient is neurologically
free, except for mild left facial paresis.
Right wide craniotomy over
the right fronto-temporal area with reflection
of the flap to the right ear. The tumor was
attacked from the temporal region then from the
mediobasal frontal lobe and all the visible by
MRI and by visual control was resected. The
fresh frozen result was glioblastoma multiforme.
Trying maximally radical removal was tried using
the anatomical landmarks. Strict hemostasis and
routine closure of the wound. Smooth
postoperative recovery.
He was sent to the ICU for 24
hours observation.
FOLLOW UP
The patient came to the clinic 10-January-2004
with clean wound and neurologically showed deep
left sided paresis.
The patient then came 15-March-2004 with new MRI
showing recurrence with involvement of the right
entire hemisphere. He was advised to perform
booster radiotherapy.
Comments
The patient has glioblastoma multiforme with
gloomy prognosis.
Skyra MRI with all clinical applications in the run since 28-Novemeber-2013.
Inomed Riechert-Mundinger System, with three point
fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
