The patient to the clinic 19-November-2002 complaining of LBP for 4 years with exacerbation of right sciatica last 2 months. MRI lumbar spine done 22-October-2002 showing huge disc L4-5 with right downward migration.

On examination: The patient in agonizing pain, limping with exaggerated scoliotic stance. SLRS 10 degrees with pain in the right and 70 degrees in the left shooting to the right. There is severe weak dorsi planterflexion right foot 3/5, with hypalgesia right S1 territory.

Right sided L5 foraminotomy, right hemiflavotomy with removal of the extrusion lateral to the axilla from the right in 2 big pieces. Right sided intradiscal cleaning L4-5. Osteophytectomy done using drilling. The patient was put in Reverse Trendelenburg position with Valsalva maneuver and hyperventilation. No CSF leak. Routine closure of the wound.

Smooth postoperative recovery. The power of the right foot improved. He was sent to the ward.

The patient came to the clinic 04-December-2002 with clean wound and normal power and sensation right foot with SLRS 80 degrees right side without pain.

The patient came several times and given sick leave and the last time came 23-February-2009 with LBP without sciatica with normal power and sensation of both feet and SLRS 70 degrees with pain left side. Treated conservatively.

There is still an estimated postoperative recurrence around 7%, because the disc space is still not shallow.

The camera man is very talkative but even not for second pay attention to the focus of the camera. In the future such person must be kept away from activity.
