PROGRESSING HYDROCEPHALUS DUE TO AQUADUCTAL STENOSIS.
Anamnesis
The patient came to the clinic 08-December-2014
complaining of occipital headache with neck pain
and blurred vision for 17 months. The patient
underwent fenestration of the floor of the third
ventricle elsewhere without any improvement. MRI
of the brain done 14-July-2013 showing still
persisting hydrocephalus. The patient receiving
L-thyroxin 100 micrograms a day since 1996.
On examination, the patient is swaying to all
directions in Romberg position. She has tinnitus
both ears. The patient hold her head when try to
set from supine position.
The patient was sent for investigations and MRI
of the brain and cervical spine done 12-December-2014
showing hydrocephalus with aquaductal stenosis.
The old bur hole refreshed in the right frontal
region. Small abdominal incision. Medtronic
regular adult type VPS inserted after checking
the patency of the device. Routine closure.
Smooth postoperative recovery.
Comments
The patient underwent endoscopic
fenestration elsewhere, trail to repeat it could put me in
similar situation with factors hidden which caused failure
of the first procedure. For what VPS was decided to perform
to avoid such possible situation.
