Anamnesis
The patient a Bahraini citizen came to the clinic

complaining of LBP for 5 months after falling
down from a horse with bilateral
sciatica down with numbness both L5 territories.
On examination: The patient in agonizing pain,
limping with exaggerated scoliotic stance.
SLRS
80 degrees both sides with pain. There is weak
dorsi and planterflexion both feet-4/5.
The patient was sent for investigations and MRI
lumbar spine performed 06-August-2002 showing
huge central disc L5-S1.
Bilateral S1 foraminotomy with removal of the
extrusion lateral to the axilla. Bilateral
intradiscal cleaning L5-S1. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.
Smooth postoperative recovery.
The power of both feet improved.
She was sent to the ward.
Follow Up
The patient came to the clinic 14-August-2002
with clean wound and full power of the feet.
Comments
There is still an estimated postoperative
recurrence around 7%.
