MULTICOMPARTMENTAL SUBDURAL HYGROMA LEFT FRONTAL AND PARIETAL LOBES.
Anamnesis
The patient came to the clinic 02-June-2007
complaining of progressing hallucination and
paraphasia. The patient was exposed to RTA
22-April-2007 and was treated for skull base
fracture with minimal subdural hematoma and
fracture C6 and left clavicle. The patient was
treated conservatively and he improved. The last
3 weeks he progressed hallucination and
paraphasia.
On examination he had mild right sided paresis.
MRI performed 03-June-2007 showing left
convexital subdural collection over the frontal
and parietal lobes.
2 burr holes were preformed over the the
posterior frontal and the parietal lobes and
hygroma was discovered. It was multilobulated
and the cavities not related to each other, for
what separated external drains were inserted for
each collection. External drain was kept for 36
hours in the ICU.
Comments
The patient has brain atrophy and it is usual
to progress chronic subdural hematoma. Here, it was a hygroma
and it was multilobulated, for what 2 external drains were used.
Check CT-scan showed resolution of the
collection, but the hallucination still having place. The right
sided hemiparesis resolved.
