EXTRUDED DISC C6-7 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 09-July-2015
complaining of neck and left upper limb pain for
20 days with numbness all fingers left hand. She
was admitted to Jordan hospital and cardio work
up was negative and after then she came to me.
The patient was operated by me more than 20
years ago for lumbar disc extrusion.
On examination; The patient has pain when
turning the head to the left and down. There is
weak grip left hand 4/5, extension same hand 3/5
and very weak left triceps muscle 2/5. There is
hypalgesia of the lateral aspect of the left
forearm and the median distribution of the left
hand. There are elements of TOS both sides.
The patient was sent for investigations and MRI
of the cervical spine showed extruded disc C6-7
with left foraminal occlusion. Dynamic studies
ruled out elements of instability.
Discectomy of C6-7 was done using the C-arm with
removal of all the extrusion until the dura was
seen all over. A 5 mm compact anatomical cage
was inserted to the disc space. Fusion of C6-7
was done using Atlantis one level 25 mm plate
with 4 4x15 mm screws. Routine closure
of the wound.
Smooth postoperative recovery. The patient got
improvement of the power of the left upper limb
and was
sent to the floor.
Comments
The recurrence rate is zero in cervical
spine disk surgery and the results are more gratifying than
the lumbar disk surgeries.
