HUGE EXTRUDED DISC L5-S1 WITH LEFT FAR DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 17-October-2015
complaining of LBP for 4 years with left
sciatica for 4 weeks with further deterioration
last week and inability to set in chair with
numbness of all toes left foot more the little
toe. He is
a known diabetic for 5 years under treatment. MRI done
14-October-2015 showed huge extruded disc L5-S1
with left far downward migration.
On examination: the patient was limping with
exaggerated scoliotic stance. SLRS was 20 degrees
with pain left side. Weak dorsiflexion left foot -3/5,,
planterflexion left foot 3/5 with absent left
AJ.
Foraminotomy left S1 root.
The root was severely compressed by the
extrusion The extrusion was removed lateral to
the axilla. Using N50, a trail to generate a pulsed mode
RF with 42 Celsius, 240 sec, 2 Hz and 20 mesec
duration the left S1 root was failed due to
technical limitations.
Routine closure of the
wound.
Smooth postoperative recovery. The power of
the left foot became normal. The patient was
sent to the ward.
Comments
The patient still has an estimated
postoperative recurrence around 7%.
