L3-4 AND L4-5.
Anamnesis
The patient came to the clinic 21-June-2003
complaining of LBP for 4 years with bilateral
sciatica more the left. MRI lumbar spine done
10-August-2000 showing stenosis L4-5. The
patient has arterial hypertension with
arrhythmia under treatment.
On examination: the patient is limping with
exaggerated scoliotic stance unable to walk more
than 10 meters. SLRS was 85 degrees left side
with pain. There is hypalgesia left S1 territory
with weak dorsiflexion both feet and
planterflexion left foot 4/5.
The patient was sent for investigations and MRI
lumbar spine done 01-July-2003 showing stenosis
L3-4, L4-5 with old extrusion L4-5 more to the
right.
Decompressive laminectomy L3
and lower half of L3 and upper half of L5 with
foraminotomy of L4 and L5 roots both sides. Left
sided discectomy L4-5 with inspection the right
side. The patient was put in Reverse
Trendelenburg position with Valsalva maneuver
and hyperventilation. No CSF leak. Epidural
transfer and free fat obtained from under the
skin was used to cover the dura to minimize the
postoperative adhesions. Routine closure of the
wound. Smooth postoperative recovery.
The power of left foot normalized. She was sent to the ward.
FOLLOW UP
The patient
then came 10-August-2003 with no sciatica and no
motor nor sensory deficit with clean wound.
Comments
There is an estimated
postoperative recurrence around 7% because the disc
space is still not completely collapsed.
