SPONDYLOLISTHESIS II DEGREE OF L4-5 WITH RESIDUAL OF DISCECTOMY L5-S1 PERFORMED
1 YEAR AGO.
Anamnesis
The patient came to the clinic 28-May-2015
complaining of LBP for 2 months with
bilateral sciatica. The patient was operated
five
years ago for PLD L5-S1 for bilateral sciatica.
MRI lumbar spine done 25-May-2015 showing
spondylolisthesis L4-5 with severe segmental
stenosis. Dynamic studies confirmed the presence
of II degree of spondylolisthesis L4-5. Cardiac
consultation was uneventful but there is
hyperthyroidism. She is allergic to Penicillin
group of antibiotics.
On examination; the patient is limping with
no scoliotic stance. Neck and upper
limbs were neurologically free. SLRS was 60
degrees with pain both sides. There is weak dorsiflexion
both feet 2/5. Weak planterflexion right foot
3/5.
Using the old incision, skeletonization of L4
and L5 and upper sacrum was achieved. Using
Legacy M.A. 4 screws 6.5x45 mm inserted to L4
and L5 bodies. Laminectomy L4 and L5 with
foraminotomy L5 roots was performed. 2 bended
rods with slight distraction and bended rods
were inserted with cross connector. The
harvested bone was melted and applied lateral to
the rods. Routine
closure of the wound.
Smooth postoperative recovery. The power of both
feet became better and the left sciatica
decreased.
Comments
The patient has spondylolisthesis and
LCS. Both problems were corrected.
