18-MAY-2004 FAREED AHMAD HMAIDI 26 YEARS
SEVERE HEAD INJURY WITH DISRUPTION OF THE VERTEBRAL COLUMN AT L1 LEVEL WITH
PARAPLEGIA.
Anamnesis
The patient suffered RTA 14-May-2004 after what
he was admitted to the ICU of Shmaisani hospital
with severe head injury and fracture dislocation
of L1 with paraplegia.
On examination, the patient
upon admission was with GCS 5/15 with unstable
vital signs. He has paraplegia below D12 with
unstable dislocated fracture L1. After 4 days of
observation, when it became clear that he will
survive open reduction and fixation was decided.
Skeletonization of D11 down
to L3 was performed. Open reduction after manual
distraction was achieved. Inspection of the dura
for tears was negative. No CSF leak. Using
Isola; DePuy-AcroMed system, transpedicular
screws inserted to L3,2 and D11 and D12 fusion
of the area was achieved. Routine closure of the
wound and smooth postoperative recovery and the
patient was sent to the ICU to accelerate his
rehabilitation. From there he was sent to
rehabilitation center.
FOLLOW UP
The patient
postoperative notes are missing, because the patient
spent his postoperative follow up with the
physiotherapy center.
Comments
Fusion in this case was performed after feeling
that the patient is recovering vital signs. For
this reason the delay of his surgery was
adopted.
