21-JULY-2007 FAREED NAHAR HASSAN 53 YEARS HUGE PLD L5-S1 WITH RIGHT DOWNWARD
MIGRATION.
Anamnesis
The patient came to the clinic 18-July-2007
complaining of LBP for 5 years without sciatica.
One month ago, after lifting heavy object got
right sciatica with numbness of the right S1
territory.
MRI performed 08-July-2007 in Qatar showed a
huge extruded disc L5-S1 with up and downward
migration right side.
On examination: the patient is limping and has
scoliotic stance with SLRS right side 75 degrees
with pain and absent AJ right side. There is
hypalgesia right S1 root territory and weak
dorsi and planterflexion right foot.
Right L5-S1 hemiflavotomy with right S1 root
foraminotomy was done. Most of the extrusion was
removed from under the axilla.
Meticulous cleaning of the disc space was
performed from the right side. The disc space
was narrow.
Smooth postoperative recovery.
Comments
The disc space was narrow, which predict a
very low rate of recurrence.
As usual, the epidural fat was transferred to
the root from the nearby structures, to prevent scar formation
around the root area.
