87 YEARS BILATERAL SUBDURAL HEMATOMAS MORE THE LEFT WITH
SEVERE SHIFTING OF THE BRAIN TO THE RIGHT WITH DECEREBRATE
RIGIDITY AND PENDING CONNING.
Anamnesis
The patient came to emergency at midnight
09-June-2025 unconscious with decerbrate
rigidity and pending conning. CT-scan done
recently showing bilateral subdural hematoma,
more the left side with shifting of the
midstructures to the right. The patient was
taken to the operating room, knowing that he was
in plavix and he has arterial hypertension and
underwent cardiac stinting previously, as life
saving measure.
Craniotomy left fronto-temporal area was done
and the rubbery tense clot was removed
accordingly and the patient was sent to the ICU
in ventilator.
The homeostasis was corrected and the
decerebration disappeared. and the patient was
put in T-piece. The gagging reflex and corneal
reflexes were noted. He could move both lower
limbs but still not responding to verbal
command. Check CT-scan done the next day
10-June-2025 showing still having subdural
hematoma in the left, but less than before and
no shifting of the midstructures.
The patient showed epiactivity for what another
CT-scan done 11-June-2026 showing growing left
hematoma.
The patient then was taken to
the operating room and the craniotomy was
refreshed and the progressing extradural
hematoma was removed and strict hemostasis to
the extradural part was achieved with waxing of
the branch of the left temporal artery and the
bone flap. The dura was opened and the
intradural rubbery clot was removed all over. It
was possible to see the entire anterior fossa
and middle fossa down to the tentorium and
lesser clinoid. There is still oozing of the
bleeding from the left parasagittal site,
which was controlled by elevating the head above
the cardiac level. Strict hemostasis was
achieved and the external Medtronic drain was
applied. Routine closure of the wound and the
patient was sent to the ICU in ventilator with
semi setting position.
FOLLOW UP
Too early now.
Comments
The case is very critical and correction
of all parameters including the brain was needed to provide
to the patient a chance for survival.
