The patient came to the clinic 12-July-2014 complaining of neck pain for 5 months with right upper limb pain for 1 year. MRI cervical spine done 01-March-2014 showing huge extruded disc C5-6 with severe spinal cord compression. and malacia of the spinal cord with right foraminal occlusion.

On examination; the patient has neck pain when looking up. Weak grip, flexion right hand and right triceps 2/5. There hypalgesia of the right hand with numbness all fingers right hand. There is also weak right leg: quadriceps 4/5, dorsi and planterflexion right foot 4/5. Dorsiflexion left foot 4/5. SLRS was 30 degrees in the right due to weakness.

The patient was sent for new investigations, which were performed 13-July-2014: MRI cervical showing huge extruded disc C5-6 with severe compression and malacia of the spinal cord with right foraminal occlusion..

The C5-6 level identified. Discectomy C5-6 with removal of all extrusion until the dura was seen at all entire resected posterior longitudinal ligament. Samarys cervical cage 17x13x6 mm was inserted to the disc space. Trestle cervical plate  one level 16 mm with with four screws fixed 4x14 were used to fuse C5-6. All stages of surgery done with C-arm control. Routine closure of the wound.

Smooth postoperative recovery. The power of the right upper and lower limbs became normal. The power of the left foot improved.

The cervical extrusion was compressing the spinal cord with malacia of the spinal cord. The morphologic changes of the spinal cord will stay permanent despite decompression.
