19-SPTEMBER-2014 FATMEH MUTHAFAR HIZAM 26 YEARS
VERY HUGE EXTRUDED DISC L4-5 WITH LEFT UPWARD MIGRATION.
Anamnesis
The patient came to the Shmaisani hospital 18-September-2014
complaining of LBP with left sciatica for 2years
with exacerbation last 2 weeks with almost drop
left foot with numbness of the left L5
territory.
On examination; the patient is limping with
exaggerated scoliotic stance. SLRS was 30
degrees with pain left side. There is almost
drop left foot with weak planterflexion left
foot 3/5.
The patient was sent for MRI of the lumbar spine
with dynamic studies. MRI of the lumbar spine
showing very huge extruded disc L4-5 with left
upward migration.
Left partial flavotomy L4-5. The upward extruded
disc was removed in one big piece. Left sided
intradiscal cleaning L4-5 disc space.
Routine closure of
the wound.
Smooth postoperative recovery. The power of
the left foot became better.
Comments
The patient still has an estimated
postoperative recurrence around 7%.
