ANAPLASTIC ASTROCYTOMA G IV RIGHT CEREBELLAR HEMISPHERE.
Anamnesis
The patient an Iraqi citizen was operated by me
04-April-2017 for huge anaplastic glioma
Grade IV. The patient then underwent radiation
and chemotherapy. Several check MRI were
negative for recurrence. The patient noticed
ataxia 1 month ago for what MRI done, showing
recurrence of the tumor in the right cerebellar
hemisphere.
Using the setting position,
the wound was opened and the same craniotomy was
reflected down. The tumor was resected according
to MRI data and the anatomical landmarks, such
as the floor of the fourth ventricle, the
aqueduct, the right foramen of Luschka and
the lateral wall of the right cerebellum. The
tentorium was full of scar, for what in was
impossible to evaluate. Insertion of SNoW
Surgicele to bed of the resected tumor. Routine
closure of the wound. The patient was sent to
the MRI suite and MRI with contrast was obtained
and showing 5% of the tumor lateral to the
cavity and under the tentorium. The wound was
reopened in the supine position and dissection
of the cerebellum from the tentorium. That part
was resected up to the tentorium. Strict
hemostasis with reapplication of SNoW Surgicele. The bone flap was
closed and routine closure of the wound
. Smooth postoperative recovery. The patient was sent to the ICU.
FOLLOW UP
Comments
Even if you leave 5% of the tumor, since you
have intraoperative MRI, do it to be 0%.
