13-SEPTEMBER-2014 FAYSAL MITLIQ AL-TAEB 55 YEARS
EXTRUDED DISC L4-5 WITH RIGHT FORAMINAL OCCLUSION. RESIDUAL AFTER LAMINECTOMY
L3,4,5 AND S1 20 YEARS AGO.
Anamnesis
The patient came to the clinic 10-September-2014
complaining of LBP with right sciatica for 4
months. The patient was operated for LCS and
laminectomy L3,4,5 and S1 was done 20 years ago.
He is using crutches for 1 week. MRI lumbar
spine done 07-September-2014 showing extruded
disc L4-5 with right foraminal occlusion.
On examination; the patient is limping with very
exaggerated scoliotic stance. SLRS was 20
degrees with pain in the right and 80 degrees
with less pain in the left. There is weak dorsi
and planterflexion right foot -3/5. The right AJ
is absent.. There is hypalgesia 15 cm above the
right knee.
Using the C-arm the L4-5 level was identified
and neurolysis of the right L5 root was done.
The extruded disc was removed in several pieces
and right sided intradiscal cleaning L4-5 disc
space was achieved.
Routine
closure of the wound.
Smooth postoperative recovery. The power of the
right foot became normal.
Comments
The patient has an estimated
postoperative recurrence around 7%.
