YEARS EXTRUDED DISC L5-S1 CENTRAL.
Anamnesis
The patient a Yemeni citizen came to the clinic

complaining of LBP for 9 months with right
sciatica down with numbness right L5 and S1 territories.
MRI done 03-August-2002 showing
extruded disc L5-S1 central with right foraminal
occlusion.
On examination: The patient in agonizing pain,
limping with exaggerated scoliotic stance.
SLRS
80 degrees right side with pain. There is weak
dorsi and planterflexion right foot 3/5,
dorsiflexion left foot 4/5.
Right S1 foraminotomy with removal of the
extrusion lateral to the axilla. Right sided
intradiscal cleaning L5-S1. The dura was very
transparent, but no defect. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.
Smooth postoperative recovery.
The power of the feet improved.
She was sent to the ward.
Follow Up
The patient came to the clinic 19-August-2002
with clean wound and full power of the feet.
The patient then came 29-October-2002 with
pocket under the wound, for what she was
admitted for investigations.
Comments
There is still an estimated postoperative
recurrence around 7%.
