YEARS EXTRUDED DISC L4-5 WITH BILATERAL SCIATICA, MORE THE RIGHT.
Anamnesis
The patient came to the clinic 17-April-2002
complaining of LBP for 2 years with bilateral
sciatica, more the right for 3 months with
numbness both big toes. MRI done recently
showing broad based extruded disc L4-5 more
prominent to the right.
On examination: The patient is in agonizing
pain, limping with exaggerated scoliotic stance.
SLRS was 25 degrees in the right with pain and
30 degrees left side with less pain. There is
hypalgesia right L5, S1 territories. There weak
dorsiflexion right foot 3/5, left foot 4/5.
Extended bilateral L5 foraminotomy
with removal of the right extrusion and
bilateral
intradiscal cleaning of L4-5. After that, the roots
became relax. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of both legs improved and she was sent to
the ward.
Follow Up
The patient came to the clinic 27-April-2002
with clean wound, sciatica free with no motor ,
nor sensory deficit. SLRS was 75 degrees both
sides without pain.
Comments
The estimated postoperative recurrence is
around 7%, because the disc space is not shallow.
