19-SEPTEMBER-2014 FAYZEH ABDO AL-TAWEEL 64 YEARS
EXTRUDED DISC L4-5 WITH LEFT FORAMINAL OCCLUSION AND DROP LEFT FOOT.
Anamnesis
The patient came to the clinic 10-September-2014
complaining of LBP with left sciatica for 1
month with drop left foot from the start with
numbness of the left foot. MRI lumbar spine
performed 04-August-2014 bad quality showing as
be left far-lateral extrusion L5-S1.
On examination; the patient is limping with
exaggerated scoliotic stance. SLRS was 90
degrees both sides. There is drop left foot with
weak dorsi and planterflexion right foot 3/5.
Planterflexion left foot 2/5.
The patient was sent for MRI of the brain,
cervical, dorsal and lumbar spine with dynamic
studies. MRI of the lumbar spine showing severe
stenosis at L4-5 with extruded disc L4-5 with
left foraminal occlusion.
Left L4-5 hemiflavotomy with foraminotomy left
L5 root. The extruded disc was removed lateral
to the axilla. Further inspection of the
foraminotomy , which was widened more inferior
to eliminate all the compressive elements upon
the root.
Routine closure of
the wound.
Smooth postoperative recovery. The
planterflexion of the both feet became normal,
but the drop left foot remained the same.
Comments
The patient has long period of time to
improve the drop left foot, because the damage of the root
is so severe.
