EXTRUDED DISC L4-5 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 25-September-2013
complaining of LBP with right sciatica for 4
years. Exacerbation of LBP with right sciatica
the last 20 days.
On examination: the patient is using crutch for
20 days, limping with exaggerated scoliotic
stance. SLRS was 30 degrees with shooting pain
in the right and 45 degrees with pain. Weak dorsiflexion right foot
4/5
and planterflexion 4/5.
The patient was sent for new MRI and done
25-September-2013 showing extruded disc L4-5
with right downward migration.
Foraminotomy right L4-5 after using C-arm. The
root was adherent to the extrusion with atrophic
changes of the root. The extruded disc was
removed lateral to the axilla. Right sided
cleaning L4-5 disc space.
Smooth postoperative
recovery. The power of the right foot became
normal.
Comments
The patient still have an estimated
postoperative recurrence rate around 7%.
