15-JUNE-2004 FAYZEH NIMER ABU-ZAYD 52 YEARS
EXTRUDED DISC L5-S1 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient a Palestinian Authority citizen came to the
clinic 09-June-2004 complaining of LBP with
right sciatica for 2 months, down to S1 root
territory. MRI lumbar spine
done 08-June-2004 showing huge extruded disc
L5-S1 with right downward migration.
On examination, the patient in agonizing pain,
limping with exaggerated scoliotic stance with
weak dorsiflexion right foot 4/5. SLRS was 20
degrees with pain right side and 45 degrees
left side with
pain.
Foraminotomy right S1
root was achieved. Right sided intradiscal cleaning L5-S1 was
performed.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. A free fat was
harvested from the
subcutaneous layer and covered the exposed parts
of the dura and roots to minimize postoperative
scarring. Routine closure
of the wound.
Smooth postoperative recovery. The power of
the right foot normalized.
FOLLOW UP
The patient
came to the clinic 26-June-2004 with clean wound and no motor,
nor sensory deficit.
Comments
There is still an
estimated postoperative recurrence around 7%,
because the disc space is still not completely
collapsed.
