EXTRUDED DISC L4-5 AND L5-S1 WITH LEFT FORAMINAL OCCLUSION BOTH LEVELS.
Anamnesis
The patient came to the clinic 16-July-2014
complaining of LBP with left sciatica for 20
years and another attack 10 years ago with
exacerbation last month with left sciatica down
to the gastrocnemius muscle.
On examination; the patient is limping with exaggerated scoliotic stance.
SLRS was 70 degrees with pain in the
right and 80 degrees in the left. There is weak
dorsiflexion left foot 4/5.
The patient was sent for investigations and MRI
lumbar spine done 16-July-2014 showing extruded
disc L4-5 and L5-S1 with left foraminal
occlusion both levels.
Left L4-5, L5-S1 hemiflavotomy with foraminotomy
left L5 and S1 roots. The extruded disc of L4-5
and L5-S1 were removed lateral to the axillae.
Left sided cleaning L4-5 and L5-S1 disc spaces. Routine closure of the wound.
Smooth postoperative recovery with dramatic
improvement of the power of left foot.
Comments
The patient still has an estimated
postoperative recurrence rate around 30% because the 2 disc spaces
are still not completely
shallow.
