YEARS EXTRUDED DISC L5-S1 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 02-January-2005 complaining of right
sciatica for 3 years with exacerbation last 3 weeks.
On examination: The patient is in agonizing
pain, limping with exaggerated scoliotic stance.
There is weak dorsiflexion right foot -4/5,
planterflexion same foot 4/5. SLRS was 80 degrees with pain
right
side. There is hypalgesia right L5
and S1 territories.
The patient was sent for investigations and MRI
lumbar spine performed 04-January-2005 showing
extruded disc L5-S1 with fragment stabbing the
right S1 root.
Extended right S1 foraminotomy
with removal of the right extrusion part subaxillary
and part Lateral to the axilla with right sided
intradiscal cleaning of L5-S1. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound. Smooth postoperative recovery.
The power of the right foot improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 16-January-2005 with dramatic
improvement with SLRS 90 degrees right side and he was
neurologically free.
Comments
There is still an estimated postoperative
disc recurrence around 7%, since the disc space is not
completely shallow.
