The patient was operated by me 02-aPRIL-2015 for diskitis of  L4-5. The patient continued to complain of agonizing LBP and septic workup failed to identify the source and cause of infection. The last MRI done 03-August-2015 regression of diskitis with reactionary changes of the endplates of L4 and L5. During this time the patient was covered with antibiotics, but as the first time he did not obey the medical advice and was shopping with several doctors, among them they gave him cortisone to the back without effect. It was explained to him that cortisone can reflair his infection and make the course more protracted. The power of both feet were normal and SLRS was 80 degrees both sides. He continued to complain of agonizing LBP more in the morning. It was suggested to him to try RF ablation using MultiGen, in the hope it will ease his condition.

Using the C-arm, the right L3,4,5 lateral processes were identified and the RF MultiGen catheters were inserted. The lateral wall of S1 was identified and the fourth catheter was inserted. Using the sensory protocol, the upper 2 catheters were most painful. The lower 2 showed sensation of current without proper pain. The motor protocol showed twitching of all the catheters. Ablation was performed with 80 degrees Celsius and 90 sec. The pain then disappeared.

The patient discharged after 1 hour.

The patient still in agonizing pain despite the 2 surgeries and long time cover with antibiotics.

RF ablation could be an appropriate option in this stage of his condition.
