The patient a Libyan citizen came to the clinic 17-February-2004 complaining of temporal lobe epilepsy with secondary generalization for 10 years with epi attacks 20-30 per day, despite using Tegretol 400 mg and Depakine chrono three times a day, without effect.

On examination: The patient has right sided hemiparesis with pathologic reflexes both sides, more brisk in the right with deep reflexes exaggerated in the right. He is right handed. MRI of the brain performed 16-February-2004 showing sclerosis of the left SMA. EEG showed epi-activity scattered more prominent at both temporal areas more the left.

Left pterional approach with reflection of the flap to the left ear. The anterior 2/3 of the temporal lobe was resected with hippocampectomy, the uncus and amygdala. All effort were paid not to violate the Broca area, nor the Wernicke's area. nor the primary and secondary auditory areas. The inferior horn of the left lateral ventricle was used to guide the resection limits. Strict hemostasis. Routine closure of the wound. Smooth postoperative recovery. The power of the right side of the body became more weak.

The patient during stay in hospital suffered from 1 attack per day. No speech motor or sensory dysfunction and during the stay in the hospital the deep right sided paresis started to improve his power stating from the lower limbs. He was advised to perform MRI and EEG after one month, but he disappeared.

Of course the patient has multi sites generating the epi attack. Another sites will expose him to permanent plegia or Kluver-Bucy syndrome.

Bringing the attacks from 30 to one attack per day is an acceptable result from such surgery.
