YEARS HUGE RECURRENT DISC L4-5 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient was operated by me
07-September-2011 for huge extruded disc
L4-5 with left foraminal occlusion. The patient
then came to the clinic 27-March-2013
complaining of right sciatica. MRI lumbar
performed 26-March-2013 showing small disc
fragment at the right side of L4-5 and was
treated conservatively. The patient then came
20-April-2014 complaining of LBP with left
sciatica for 7 weeks. MRI lumbar spine done
29-March-2014 showing huge recurrence of L4-5
with left foraminal occlusion.
On examination; the patient is limping with
no scoliosis. SLRS was 40
degrees left side with pain. There is drop left
foot. MRI with contrast and dynamic studies
confirmed the previous data.
The old incision was
refreshed. Slight drilling of the left upper
corner of the previous laminectomy site.
Neurolysis of the left L5 root. The extruded
disc was severely compressing the root and
massive adhesions were noted. The disc material
was pushed inside the disc space piece by piece
to minimize the surgical trauma to the L5 root.
Complete removal of the extrusion was achieved
and the nerve was hanging free at the end of
surgery.
Smooth postoperative
recovery. The power of the left foot slightly
improved.
Comments
The patient has a zero estimated postoperative
recurrence, because the disc space was very
shallow and empty.
