YEARS L3-4 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 04-August-2015
complaining of LBP with left sciatica for 3 weeks with numbness of
big toe left foot. MRI lumbar spine done
02-August-2015 showing extruded disc L3-4 with
left downward migration with bulge L4-5.
On examination: He was limping, with no scoliotic stance.
SLRS was 40 degrees with pain left
side. There is weak dorsiflexion right foot
3/5.
Using the C-arm the L3-4 level was identified.
Foraminotomy
left L4 root. The extruded downward migrating
piece was removed in one piece. Discectomy L3-4 from
the left. The root became lax and free. Routine closure of the
wound.
Smooth postoperative recovery. The power of the
left foot became normal.
Comments
The patient still has an estimated
postoperative recurrence around 7% because the disc space
still not completely shallow.
