09-JULY-2003 HADEEL JAMAL ABU-AL-NADI 28 YEARS
OLD EXTRUDED DISC L5-S1 LEFT SIDE.
Anamnesis
The patient came to the clinic 07-July-2003
complaining of LBP with left sciatica for 3
years with exacerbation last 18 months. MRI lumbar spine performed
24-June-2003 showing extruded disc L5-S1 left
side.
On examination, the patient limping with exaggerated
scoliotic stance. SLRS was 35 degrees with pain
right side and 15 degrees with more pain left
side. There is weak planterflexion left foot 4/5
with absent AJ left side.
Left L5-S1 hemiflavotomy with
foraminotomy left S1 root. The extruded disc was
removed and the bony parts were drilled off.
Left sided intradiscal cleaning of L5-S1 disc
space was achieved. Fat transferred over the
very transparent dura. The
patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. Routine closure of the wound.
Smooth postoperative recovery.
The patient showed normalization of the power of
the left leg. She was sent to the ward.
FOLLOW UP
The patient
came to the clinic 20-July-2003 with clean wound.
The power of left foot was normal. SLRS was 30
degrees left side with pain.
The patient
then came 02-September-2003 with further improvement:
SLRS was 60 degrees with less pain in the left.
Comments
There is an estimated postoperative recurrence
around 7%, because the disc space is not
shallow.
