18-JUNE-2014 HAIFA DAWOOD AMRO 53 YEARS
EXTRUDED DISC C6-7 WITH RIGHT FORAMINAL OCCLUSION AND TO LESSER DEGREE AT C4-5.
Anamnesis
The patient came to the clinic 16-June-2014
complaining of neck and right upper limb pain
for 1 week with pain and numbness of the ulnar
distribution right upper limb. The patient
suffered RTA 4 years ago with fracture of the
neck? MRI cervical spine done 14-June-2014
showing extruded disc C6-7 with right foraminal
occlusion and to lesser degree at C4-5. EMG of
both upper limbs done 15-June-2014 reported as
normal. The patient has bronchial asthma for 20
years using Symbicort 2-3 times during the
month.
On examination; The patient has pain with
limitation of movement when
looking up and to both directions more to the
right and bending the head to the
right shoulder. There is weak grip right hand
3/5, extension of the fingers 3/5 and left
triceps muscle 2/5.
Using the C-arm, the C6-7 level identified.
Discectomy C6-7 with special attention to the
right side to remove all inside extrusion.
Samarys cervical cage 17x13x6 mm inserted to the
disc space. Trestle cervical plate 16 (22 mm)
length was used with 4 fixed screws to achieve
fusion C6-7. All stages were done with C-arm
control. Routine closure of the wound.
Smooth postoperative recovery. The power of the
right upper limb became normal.
Comments
The patient has 2 extrusions, one of them
needs surgery and the other not justified.
