18-DECEMBER-2014 HAJER WAFIQ SAFADIYE 63 YEARS
EXTRUDED DISC L4-5 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 13-December-2014
complaining of LBP for 4 years with right sciatica
for 8 months with exacerbation of the LBP with
numbness big toe right foot. MRI
lumbar spine very bad quality done 19-May-2014 showing
as be an extruded disc L4-5 with right foraminal
occlusion with upward migration. The patient is
a known hypertensive for 2 years in treatment,
diabetic for 15 years, insulin dependent and
have rheumatoid arthritis for 18 years in
prednisolone and methotrexate 6 tab every
Friday.
On examination, the patient is using crutches
for 8 months, limping walking bended anterior with exaggerated scoliotic
stance. SLRS was 70 degrees with pain in the
right. There is weak dorsiflexion
right foot -4/5.
The patient was sent for investigations and MRI
lumbar spine done 14-December-2014 showing
extruded disc L4-5 with right foraminal
occlusion with bulge L3-4.
Using C-arm the level of L4-5 was identified and
foraminotomy right L5 root was performed. The
extruded disc was removed lateral to the axilla
and right sided intradiscal cleaning was done.
To promote the healing since she was receiving
Methotrexate, 5 cc of her blood was centrifuged
and applied to subdermal layer during closure.
Smooth postoperative recovery. The power of the
right foot became normal.
Comments
There is still an estimated postoperative
recurrence around 7%.
