YEARS SEVERE LUMBAR CANAL STENOSIS L4-5 WITH POSSIBLE SEGMENTAL MOBILITY.
Anamnesis
The patient came from Libya 09-September-2014
and admitted to Shmaisani hospital
complaining of LBP with left sciatica for 3
years with bilateral sciatica with
numbness both lower limbs at L5 and S1
territories. MRI
done 09-September-2014 showing severe lumbar
canal stenosis L4-5. The patient is a
Parkinsonian patient for 1 year without
treatment.
On examination; the patient is bedridden with
Foleys catheter. She was using wheelchair for 6
months. She has tremor four limbs more the left
side. She had chronic constipation for long time
and high enema was needed to ease her abdominal
pain. SLRS was 30 degrees both sides with pain.
Weak dorsi and planterflexion both feet 2/5 with
analgesia both L5 and S1 territories.
Senimet 250/25 1/2 tab 3 times a day was started
and blood transfusion was given before surgery
to correct the anemia.
Decompressive laminectomy lower half of L4
and upper half of L5. Foraminotomy L5 roots
both sides. All the compressive elements were
eliminated. Routine
closure of the wound.
Smooth postoperative recovery. The power of the
feet still the same.
Comments
The patient has severe weak both feet
within long period of time. The sooner the intervention, the better the recovery.
