Anamnesis
The patient came to the clinic 10-January-2005
complaining of LBP with bilateral sciatica for
10 years with exacerbation the last year. He can
walk only 10-20 meters with intermittent
claudication. He is a known diabetic for 12
years. Dripping urine for 3 months. MRI lumbar
spine performed 22-May-2004 showing severe
lumbar canal stenosis L3-4 and L4-5.
On examination: The patient is in pain, limping with exaggerated scoliotic stance.
There is drop both feet with weak planterflexion
more the left foot. SLRS was 80 degrees both sides.
There is hypalgesia 20 cm above the ankles both
legs.
Extended decompressive
laminectomy L,4, upper half of L5 and lower half of
L2. Foraminotomy both L4 and L5 roots. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the feet still the same.
She was sent to the ward.
Follow Up
The patient came 16-March-2005 with slight
improvement of the power of the feet and
anesthesia of the feet at L5 territories
,feeling the dorsum of the feet.
Comments
The patient came with irreversible stage
of neural damage. The aim of surgery to stop escalation of
other complications.
