28-MAY-2014 HALIMEH MUHAMED ALIYAN 60 YEARS
EXTRUDED DISC L5-S1 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 01-May-2014
complaining of right sciatica with LBP for 2
years with numbness right foot.
On examination; SLRS was 90
degrees in the right with pain. There is weak
dorsi and planterflexion right foot 4/5 with
hypalgesia right L5 root territory.
The patient was sent for investigations. MRI
lumbar done 05-May-2014 showing bulge disci
L1-2, with stenosis L2-3, L3-4 and L4-5 with
spondylolisthesis L2-3 and L3-4. There is an
extruded disc L5-S1 with right foraminal
occlusion.
Laminectomy of L5 with lower half of L4 with
foraminotomy right L5 and S1 roots. The extruded
disc of L5-S1 was bony hard and severely
compressing the right S1 root. It was necessary
to drill off the extrusion, so as to achieve
sufficient decompression. The disc space of
L5-S1 was very shallow and empty.
Smooth postoperative recovery. The power of
the right foot became normal.
Comments
The patient has many problems with
stenosis of mild degree with spondylolisthesis of different
levels. In this case it is preferable to deal with the
essential problem, which is causing the clinical
manifestations, which is the extruded disc at L5-S1.
During surgery, it was decided to
eliminate the severe stenosis at L4-5.
The estimated postoperative recurrence is
around 0%, because the disc space is severely
shallow and empty.
