Comments
The sooner surgical decompression, the
better the outcome in progressing lumbar canal stenosis.
Anamnesis
The patient came to the clinic 24-February-2003
complaining of inability to walk with numbness
both feet for 3 weeks. She was operated for PLD
L5-S1 01-April-1985. MRI lumbar spine performed
18-February-2003 showing severe lumbar canal
stenosis L4-5 with possible extrusion. Diabetic
for 17 years, hypertensive for 20 years.
On examination: the patient in pain,
limping with exaggerated scoliotic stance. SLRS
was 30 degrees with pain both sides. There is
drop both feet and severe weak planterflexion
both feet, diabetic neuropathy with analgesia
left S1 territory.
Decompressive laminectomy
L4-5 with foraminotomy both L5 roots. There was a
dural defect at the right lower corner, which was
stitched by 6 zero nylon. Inspection of the disc of
L4-5 was uneventful, for what it was decided not to
violate it. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine closure of
the wound. Smooth postoperative recovery.
The radicular pain decreased and the power of both
feet still the same. She was sent to the ward.
Follow Up
The patient came to the clinic 19-March-2003
with clean wound. Still having drop right foot
with weak dorsiflexion left foot 4/5.
Planterflexion booth feet normalized. Sensation
improved.
