The patient came to the clinic 08-February-2014 complaining of neck pain radiating to the right upper limb for 45 days.

On examination; the patient in agonizing pain with inability to move the neck to all directions with more pain when looking to the left. There is weak grip right hand -4/5, extension right hand -3/5 and right triceps -3/5. There is hypalgesia of the ulnar side of the arm, forearm and the hand.

The patient sent for MRI of the cervical spine and done the same day, showing extruded disc C5-6 with right foraminal occlusion.

Discectomy C5-6 was done until the dura was seen at the entire posterior aspect so as not to miss any inside extrusion. Vitatech secured cage 5.5x16x13 mm with Hydr,Os bone graft was inserted to the disc space. I level cervical plate 21 mm with three screws 4x16 mm and one revision screw 4.5x16 mm were used to fuse C5.6 level. The revision screw was inserted to the right lower part of the construct. All stages were performed with the C-ram control. Routine closure of the wound.

Smooth postoperative recovery. The patient showed dramatic improvement of the power of the right upper limb and disappearance of the radicular syndrome.

The cage is a secured type, but it is not reliable. For that more fixation was done to achieve proper fixation with stable construct.
