26-FEBRUARY-2012 HANA IBRAHEEM MUSTAFA 53 YEARS
EXTRUDED DISC L2-3 WITH RIGHT DOWNWARD
MIGRATION.
Anamnesis
The
patient came to the clinic
25-February-2012 complaining of LBP for 20 years
with right sciatica for 5 months with numbness
of all toes right foot and exacerbation the last
month.
MRI lumbar spine done 01-February-2012 showing
extruded disc L2-3 with right far downward
migration.
On
examination, the patient is limping with
exaggerated scoliotic stance. SLRS was 90
degrees in the right with pain. There is weak
dorsiflexion right foot -4/5. The right KJ is
absent.
The L2-3 level was identified
by the C-arm. Extended foraminotomy of the right
L3 root was done. The far downward migrating
disc fragment was removed in several pieces.
Right sided cleaning of the disc space of L2-3
was done. The distorted annulus fibrosis was
removed also.
Routine
closure of the wound. Smooth postoperative
recovery with normalization of the power of the
right foot.

Comments
The patient has had far-down migrating
disc which was separated from the disc space, for what SLRS
was 90 degrees.
The estimated postoperative recurrence
still around 7%, because the disc space is still not
shallow.
