19-FEBRUARY-2014 HANI HELMI AL-WADI 40 YEARS
EXTRUDED DISC L5-S1 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 11-February-2014
complaining of LBP with left sciatica for 11
days. Yesterday go an agonizing sciatica.
MRI lumbar spine done 03-February-2014 showing
bulge L5-S1.
On examination; the patient in agonizing pain,
limping with exaggerated scoliotic stance. SLRS
was 30 degrees with pain left side. AJ was
absent in the left side. Weak dorsiflexion left
foot 3/5 and planterflexion same foot 4/5.There
is hypalgesia left L5 and S1 territories.
The patient sent for MRI of the lumbar spine
and done 12-February-2014, showing extruded disc
L5-S1 with left downward migration.
Left S1 foraminotomy with removal of the
extrusion lateral to the axilla. Left sided
intradiscal cleaning L5-S1 disc space.
Smooth postoperative
recovery. The power of the left foot became
normal.
Comments
There is still an estimated postoperative
recurrence around 7%, because the disc space is
still not completely shallow.
