29-DECEMBER-2003 HANIFEH JAMEEL BAYER 65 YEARS
EXTRUDED DISC L2-3 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 24-December-2003 complaining of
LBP with right sciatica for 2 months with
numbness of the left foot. MRI lumbar spine done
22-December-2003 showing huge extruded disc L2-3
with right downward migration. The is diabetic
for 4 years and hypertensive for 24 years in
treatment. She performed thyroidectomy 12 years
ago in L-thyroxin 100 microgram daily.
On examination: The patient is limping with
exaggerated scoliotic stance. SLRS was 80 degrees in the
right with pain. There is
weak dorsiflexion and
planterflexion right foot 3/5 with hypalgesia
right L5 and S1 territories.
Decompressive laminectomy
L2-3 with right L3 foraminotomy with
removal of the extrusion. After
that, the root became lax and right sided
intradiscal cleaning L2-3 disc space was done.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. A free fat was
harvested from the subcutaneous layer and
covered the exposed parts of the root. to
minimize postoperative scarring. Routine closure
of the wound. Smooth postoperative recovery. The
power of the right foot became normal.
FOLLOW UP
The patient came to the clinic 10-Januuary-2004
with clean wound, but exudate under the skin and SLRS 80 degrees left side
without pain and no motor, nor sensory deficit.
The patient then came 28-March-2004 and
03-February-2005 complaining of hip pain and the
last time felt in the right knee 9 days ago and
was treated conservatively.
Comments
The estimated postoperative recurrence is still
ranking around 7%, because the disc space is
still relatively not shallow.
