There is still an estimated postoperative re-recurrence around 7%, because the disc space still not shallow.

The patient was operated elsewhere 29-June-1999  for PLD L4-5 for right sciatica. Then he came to the clinic 30-April-2002 complaining of left sciatica for 10 days and MRI lumbar spine performed 28-April-2002 showing recurrence of L4-5 with left foraminal occlusion.

On examination: The patient in agonizing pain, limping with scoliotic stance. SLRS was 45 degrees with pain left side. There is weak dorsi and planterflexion both feet -4/5, weaker the right with hypalgesia right S1 territory.

Neurolysis of the left S1 root, foraminotomy right S1 root and the extruded disc was removed lateral to the axilla. Bilateral intradiscal cleaning of L5-S1 disc space. After that, the roots became relax. There were 2 tiny tears at the posterior aspect of the dura and the lateral aspect of the lateral wall of the right L5 axilla which were repaired by nylon. The patient was put in Reverse Trendelenburg position with Valsalva maneuver and hyperventilation. No CSF leak.  Routine closure of the wound.

Smooth postoperative recovery. The power of the left foot improved. He was sent to the ward.

The patient came to the clinic 27-May-2002 with clean wound , SLRS 80 degrees with pain and no motor deficit and hypalgesia right S1 territory.

The patient then came 14-September-2013 with right sciatica for 2 weeks with new MRI done the same day, showing no recurrence at L4-5, but small extrusion at L2-3 right side. He was limping with exaggerated scoliotic stance with slight weak planterflexion right foot 4/5. There is hypalgesia of the right S1 territory.
