17-JULY-2003 HASAN ALI ABDEL-HADY 60 YEARS
POST-TRAUMATIC PCD C3-4 WITH CONTUSION OF THE SPINAL CORD.
Anamnesis
The patient came to the clinic 10-July-2003
in wheelchair complaining of tetraparesis and
loss of micturition control, in Foley's
catheter. The patient after RTA 26-June-2003 got
this condition.
On examination, the patient in wheelchair with
Foley's catheter with profound tetraparesis
below C3 level with hypalgesia more profound in
left side. All pathologic reflexes present with
clonus.
The patient was sent for investigation and MRI
of the cervica spine showed extruded discC3-4
with severe compression of the spinal cord and
malacia of the spinal cord.
Trans-nasal intubation with
anterior approach. The C-arm was used to verify
the C3-4 level. Discectomy of C3-4 with
osteophytectomy until the dura was seen. It was
adherent with the posterior longitudinal
ligament, for what a tiny layer was left in
place to avoid rupture of the dura. Routine closure of the wound.
Smooth postoperative recovery.
The power of four limbs slightly improved. He was sent to the ward.
FOLLOW UP
The patient
came to the clinic 27-July- 2003 with clean wound
and still in wheelchair with slight improvement of
the power of upper limbs, but the power of the lower
limbs improved to lesser degree. Still using Foley's
catheter. He was advised to undergo physiotherapy.
Comments
The tetraparetic patient in
wheelchair rarely improve to the degree that he can
walk alone without aid. Age playing a great factor
in this situation.
