19-JUNE-2003 HASAN HASAN AL-KOOZ 65 YEARS
JEFFERSON FRACTURE AND CRUSHED D4.
Anamnesis
The patient came to the clinic 05-May-2003
complaining of LBP for 5 years with exacerbation
last 2 months with right sciatica. The patient
was sent for investigations, but he felt down
19-June-2003 and admitted to Shmaisani hospital
with Jefferson unstable fracture and crushed
dislocated D4 with complete paraplegia below D3
with severe weak both upper limbs and loss of
sensation below the nipples.
The patient was operated
through 2 separate incisions. Using Harrington
distraction rods, distraction-reduction was
achieved with sublaminar hooks between D6 and
D1. After closure of the wound, skeletonization
of C3,2,1 and occiput was achieved and bended
Luque rectangle adopted to regain the natural
curve of the area with sublaminar wires fusion
of the area was achieved.
Smooth postoperative recovery.
He was sent to the ward.
FOLLOW UP
The patient
came several times and the last one came 31-July 2005
with recovery of the power and sensation of the
upper limbs with the analgesia 10 cm below the
nipples.
Comments
The patient has complete paraplegia below D3. It
is hard to believe that he will recover this
segment, but the weak upper limbs for sure will
recover.
