The patient came to the clinic 06-September-2012 complaining of left shoulder pain for 45 days with minimal neck pain. The pain then increased to involve the ulnar territory of the left upper limb.

MRI cervical spine done 16-August-2012 showing PCD C6-7 with left foraminal occlusion.

On examination, the patient in agonizing pain  and has pain when looking to the left and bending the head to the left shoulder and tightness when looking down. There is weak grip and extension left hand 4/5 and the left triceps 3/5.

MRI repeated 08-September-2012 showing the same data and a small extrusion in the right side at C6-7 level.

Discectomy of C6-7 with osteophytectomy until the dura was seen from the most lateral aspects both sides. Fidji cage 12x17x5.3 mm was inserted to the disc space with Novabone. Using Trinica plate 22 mm one level and 4 screws 14x4.2 mm fixed type were used to fuse C6-7. All stages of surgery were performed using C-arm.

Routine closure of the wound. Smooth postoperative recovery and the power of the left upper limb became normal.

The patient has extruded disc with foraminal occlusion. Surgical decompression with stabilization is the appropriate solution and recurrence in this case is zero.
