The recurrence rate in this case still ranking around 7% because the disc space is not completely collapsed.

The patient came to the clinic 01-February-2003 complaining of LBP with left sciatica for 4 years with intermittent course. The last month right sciatica. Diabetic for 7 years in medication. MRI lumbar spine performed 22-January-2003 showing huge extruded disc L4-5 more to the right.

On examination: the patient in pain, limping with exaggerated scoliotic stance. SLRS was 80 degrees with pain both sides. There is almost drop right foot with weak planterflexion right foot -4/5. Hypalgesia right L5 and S1 roots.

Decompressive laminectomy L4-5 with extended foraminotomy both L5 roots. The extruded disc of L4-5 was attacked lateral from the right axilla and bilateral cleaning of L4-5 was achieved until the roots became lax. The patient was put in Reverse Trendelenburg position with Valsalva maneuver and hyperventilation. No CSF leak. Routine closure of the wound

Smooth postoperative recovery. The radicular pain disappeared and the power of the right foot normalized. She was sent to the ward.

The patient came to the clinic 07-May-2003 with clean wound with some reaction around the wound to the plaster with no motor deficit. There is numbness of the big toe right foot. SLRS was 80 degrees without pain.

The patient then came 04-June-2003 complaining of left sciatica for 2 weeks with no motor nor sensory deficit. She was treated conservatively.

The patient then came complaining of left sciatica  and sent for check MRI which was done 15-September-2005 ruling out any recurrence. She was treated conservatively.
