YEARS LCS L4-5, L5-S1. RESIDUAL AFTER LYSIS OF THE CYSTIC SPINAL CORD AND
DISCECTOMY.
Anamnesis
The patient a doctor came to the clinic
13-March-1999 complaining of LBP with left drop
foot and sent for investigation. He disappeared
and came back 10-February-2001 telling that in
Islamic hospital was operated for cystic spinal
cord and discectomy was done and having drop
left foot. The patient then sent for
investigations and disappeared to come back
20-April-2003 with drop left foot with new
MRI done 10-April-2003 showing recurrence of
extruded disc L4-5 left side with stenosis L4-5
and L5-S1 and was advised for surgery, but he is
hesitating.
Decompressive laminectomy
L4-5 and L5-S1. There was massive scar in this
area and scarolysis with foraminotomy both L5
and S1 were achieved both sides. The bony
extruded disc L4-5 was drilled away and left
sided intradiscal cleaning L4-5 was achieved.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. Routine closure
of the wound.
Smooth postoperative recovery. The power of the
left foot slightly improved.
FOLLOW UP
The patient
came to the clinic 20-September-2003 with clean wound
and the power of the left foot was 4/5.
The patient
then came 14-June-2004 after falling down several
months ago with LSS X-ray showing healed fracture
L4. He was treated conservatively.
Comments
The patient mostly
not telling the details of his surgery or
surgeries, but why a drop foot for more than 3
years recovered so quickly. I cannot understand
that.
