02-JULY-2014 HISHAM YOUSEF GHAWANMEH 46 YEARS
LEFT FAR-LATERAL EXTRUSION L4-5 WITH SEVERE FORAMINAL OCCLUSION.
Anamnesis
The patient admitted urgently to Shmaisani
hospital at 2.00 A.M. 01-July-2014 with
agonizing LBP with left sciatica. There is
numbness of the anterior left thigh and the
perianal region. The patient is unable to set or
walk or stand for examination. Pethidine was
given to ease his pain. The pain started several
hours ago.
On examination; the patient was unable to
examine him for limping or scoliotic stance
evaluation. SLRS was
0 degrees
both sides. There is weak dorsi and
planterflexion left foot 3/5.Hypalgesia
left L5 and S1 roots territories.
The patient was sent for MRI lumbar spine which
showed left far-lateral extrusion L4-5 with
foraminal occlusion.
Using MICRODISCXS system with VITOM, left L5
foraminotomy was done and the left far-lateral
extruded disc was removed in several pieces.
Left sided intradiscal cleaning L4-5 disc space.
Smooth postoperative recovery.
The power of the left foot became normal.
Follow up
The wound was small but strangely oozing. He was
covered with Cefazolin, which was stopped
04-July-2014 and the patient was discharged
05-July-2014.
Comments
The estimated postoperative recurrence is
still around 7% because the disc space height is still not
shallow.
