The patient has LBP for 18
months with tolerable pain and left sciatica. The last 14 days he
got very severe left sciatica and inability to walk and scoliotic
stance with almost drop left foot and hypalgesia left L4 territory.
MRI showed huge PLD L3-4 with left downward migration of the disc
material. The patient was operated and left L3-4 hemiflavotomy and
left L4 foraminotomy was performed. Most of the extrusion was
sequestrated and removed from under the axilla. Mild cleaning of the
disc space was performed. The canal for the left L4 root was
inspected.
